**Notice of Collection** *Personal information on this form is being collected for the purpose of administering applications to the City of Toronto’s Road Disruption Activity Reporting System (RoDARS) that informs the public of planned roadway closures throughout the City. Personal information is collected for RoDARS under the authorities of sections 8(1), 8(2)(paras, 5, 6), 32(1), and 259 of the City of Toronto Act, 2006, Section 743-30.1, Article III.1, Chapter 743 (Streets and Sidewalks, Use of) of the City of Toronto’s Municipal Code, and in accordance with recommendation 38 of Toronto City Council Item CC27.1 adopted by City Council on February 11, 2025 (By-law 108-2025). This new RoDARS Application form is for use by contractors working for members of the Toronto Public Utility Coordination Committee (TPUCC) such as Enbridge, Rogers, Bell, Toronto Hydro, Telus, etc. Utility maintenance work for Toronto Water and TTC are also considered as utility clients that must use this form. Companies doing work for Transportation Services, traffic signal maintenance should also be using this form.   Before applying for a permit or RODARS, all utility companies must have a pre-approved client code set up through the Utility Cut Permit group for the permit and billing process. Additional Conditions and Comments

Any changes to the time and space occupied after approval must be requested, using the Minor Amendment button that appears once your application is approved. This includes early completions! Photos are to be uploaded before and after occupation, and in some cases during, using the Upload Site Photo button that appears once your application is approved. See Conditions for the specifics.

Next, enter the Location Description, Location Type, and Duration for each location, and then "Save" each location! Once you have confirmed your road segment(s) is open, you may wish to use the "None" map options to make it easier to draw your locations without icon interference.
