**Notice of Collection** *Personal information on this form is being collected for the purpose of administering applications to the City of Toronto’s Road Disruption Activity Reporting System (RoDARS) that informs the public of planned roadway closures throughout the City. Personal information is collected for RoDARS under the authorities of sections 8(1), 8(2)(paras, 5, 6), 32(1), and 259 of the City of Toronto Act, 2006, Section 743-30.1, Article III.1, Chapter 743 (Streets and Sidewalks, Use of) of the City of Toronto’s Municipal Code, and in accordance with recommendation 38 of Toronto City Council Item CC27.1 adopted by City Council on February 11, 2025 (By-law 108-2025). This application is to be completed if the work is initiated by a Utility company and is being completed on assets that are Utility owned only. Examples include work being completed on behalf of utility companies such as Enbridge, Hydro, Bell, Rogers etc.

Any changes to the time and space occupied after approval must be requested, using the Minor Amendment button that appears once your application is approved. This includes early completions! Photos are to be uploaded before and after occupation, and in some cases during, using the Upload Site Photo button that appears once your application is approved. See Conditions for the specifics.
